School Board Candidates for the Nov. 2, 2021 Election

The election paperwork and candidate guide for the upcoming November 2, 2021 election are linked below.  Election paperwork must be filed with Jeff Swartzentruber, Business Manager/Board Secretary.  Questions can be submitted to him via email at or by phone at 319-646-6093.

Due dates are: 
The first day that paperwork can be filed is Monday, Aug. 23
The last day that paperwork can be filed is Thursday, Sept. 16 at 5 p.m.

Candidates need to obtain at least 50 signatures using ONLY the Nomination Petition form linked above.  Please do not use any other form.  The Auditor's office encourages candidates to obtain more than 50 signatures in case signatures are challenged and subsequently thrown out.

For completion of the Affidavit of Candidacy:

  • This is a City/School Election on 11/2/2021
  • The Candidate Affiliation is BLANK
  • It must be signed in the presence of a Notary Public.  MPCSD Human Resources Coordinator, Carmela Ulin, is a Notary.

For your reference, below are last year's documents published by Washington County, Johnson County, and Iowa County with the official information of the election.  Included are the candidates for the position of School Board, and two (2) Public Measures:  (1) VF is the Public Measure to approve the extension of the Property, Plant, and Equipment Levee (PPEL); and (2) VG is Public Measure to approve the adoption of a Revenue Purpose Statement for the Secure and Advanced Vision for Education Fund (SAVE).  These measures have been reviewed and approved by the Mid-Prairie School Board to be included for voters to consider for passage.
