3-Year Old Preschool Electronic Enrollment is happening at MPCSD Preschools from April 22, 2020 to May 15, 2020!
Please complete the form below and submit if you have a child whom you are enrolling for our 3 Year Old Preschool program for the 2020-2021 school year (student turns 3 years old on or before August 15, 2020).
By filling out this form and submitting, you are reserving a spot in a 3 year old preschool program with the Mid-Prairie Community School District.  Your registration will not be complete until you finalize it during e-registration prior to the beginning of school.  You will receive further notifications on e-registration in the summer.
Please fill out the form completely!  District transportation is NOT PROVIDED for students in our 3 year old program.  
Placement of students will be at Mid-Prairie West Elementary in Wellman, IA.  3 Year Old Preschool sessions are in the AM from 8:15-11:15 AM and will be either Mondays & Thursdays OR Tuesdays & Fridays.  There is no 3 year old preschool on Wednesdays.
3 Year Old Preschool is a tuition-based program and will be $110 per month due by the 5th of each month starting in September 2020.
